You may make your appointment either in person, by phone, or by going through a third party.  When making appointments by phone, please describe your health problems, supply your PESEL (Personal Identification Number), name and surname, address and phone number. 
 
Cancer patients or patients wishing to confirm / exclude suspicion of cancer do not require any referral.  Patients making appointments to see an endocrinologist must present a referral issued by their family doctor (referral to an outpatient endocrine clinic).

If you cannot get through during our morning business hours, please call us in the afternoon.  The reception desk is open until 5 PM.    
You will need the following documents to make your appointment:
  • ID card or other identity document (passport, driving license) used by the eWUŚ electronic system to verify your health coverage;
  • Medical documentation from previous treatments: hospital discharge documentations, consultations, histopathology tests, diagnostic documentation;
  • Written referral to an outpatient clinic or for diagnostic tests (if needed).

Supplementary documents:
  • Health insurance coverage document

Appointment cancellation

Please inform our front desk if you need to cancel your appointment.  Keep in mind it may be some time before you can get another appointment. 
Patients entitled to immediate consultations:
  • Emergency cases
  • Volunteer blood or bone marrow donors
  • Disabled war veterans
  • Retired military personnel and victims of repression
  • Qualified active military personnel and workers (treatment of injuries and diseases contracted during foreign service)
  • Other veterans (treatment of injuries and diseases contracted during foreign service)
