Access to Medical Documentation

Medical documentation of the Institute of Oncology is available to
  • Patients or their legal representative,
  • Individuals authorized by the patient in writing,
  • Other entities authorized by law.
The medical documentation (patient medical history) remains the property of the Institute of Oncology and only authorized personnel may make it further available.  In the case of death, access to the deceased patient’s medical documentation is available only to the individual the patient authorized in writing prior to his/her demise.
Medical documentation is available only under conditions assuring confidentiality and personal data protection.

How to obtain a copy of medical records?

Patients, their legal representatives or individuals duly authorized by the patient in writing may obtain copies of medical records upon a written request.  Please fill out the application form and mail it to the Institute address or leave it at the Medical Archives (Room 0.040), at the Registration desks or respective front desks.

Where to go to pick up your medical records?

To pick up your copy of the medical records, please go to the Medical Archives (Room. 0.040); office hours are 7:00 AM to 6:00 PM; or you may request the documentation be mailed to you. 

Under the Patient Rights and the Office of Patient Rights Ombudsman Act of November 6, 2008 (Journal of Laws [Dz.U.] No 2012/156 uniform text), the Institute of Oncology may charge fees for making copies of the medical records.  The fee does not include postal charges which will be added separately.
